FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to surgical ophthalmology, and can be used for implantation of Ahmed antiglaucoma drainage in case of neovascular angle-closure glaucoma and pseudophakia. The bulbar conjunctiva is incised along the limbus, the conjunctival flap is formed between the two rectus oculomotor muscles, the tenon capsule is cut off from the episclera with the formation of a conjunctival pocket, the drainage tube is washed with saline, the drainage body is installed in the formed conjunctival pocket and the drainage body is fixed to the sclera with two interrupted sutures. In patients with pseudophakia, ultrasound biomicroscopy of the anterior segment of the eye is performed before surgery. The size of the posterior chamber of the eye is measured from the iris to the intraocular lens, and if its size is more than 2 mm, a drainage tube is implanted into the posterior chamber as follows: a L-shaped incision of the sclera is made at the limbus for its thickness. In this case, one side of the incision is perpendicular to the tangent to the limb 4 mm long, the other side is parallel to the limb 4 mm long. A scleral flap is formed 3 mm from the limbus under the scleral flap with a 23 G needle. An entrance channel is formed into the posterior chamber. The drainage tube is cut at an angle of 30 degrees from the side and inserted into the posterior chamber of the eye. Three interrupted sutures are placed on the scleral flap. Next, the anterior chamber is filled with viscoelastic to normal tone, and the conjunctival flap is fixed with interrupted sutures.
EFFECT: method reduces the risk of intraoperative hemorrhagic complications and prevents the development of corneal dystrophy in the postoperative period due to the claimed technique of Ahmed antiglaucoma drainage implantation.
