SUBSTANCE: invention refers to medicine, namely to surgery and oncology and can find application in removal of lymph nodes and subcutaneous fat of anterosuperior mediastinum, e.g. in metastases of thyroid cancer. Substance of the method consists in removal of lymph nodes and subcutaneous fat of anterosuperior mediastinum to tracheobroncheal angles, bifurcation of aorta, along precava and its branches with using video-assisted endoscopic tools. The wide surgical approach to mediastinal lymph nodes is ensured by dissection and separation of sternoclavicular joints. Ligation of thoracic lymphatic duct is followed with monitoring and neurolysis of recurrent nerves, layered separation of subcutaneous fat with lymph nodes and removal thereof.

EFFECT: application of the present invention allows considerably reducing number of intra- and postoperative complications, preserving a thorax skeleton, providing adequate survey of anterosuperior mediastinum structures, ensuring prevention lymphorrhagia and damages of recurrent laryngeal nerves in mediastinum that in combination leads to reduced time of postoperative recovery, improved cosmetic effect of operation, restored work capacity.
