FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to traumatology and orthopaedics. Skin and subcutaneous fat are incised on the anterior iliac region. Muscular tissue is immobilised in a blunt and sharp way. The iliac periosteum is separated between the anterosuperior and anteroinferior spines. The periosteum is dissected longitudinally and separated with a raspatory. Two tunnels in a frontal plane are drilled. Thereafter, threaded nails 5-6 mm in diametre are driven in at depth 3-4 cm. Further, in the lower one-third of femur, two notches are made with a scalpel. A trocar is inserted to reach a bone. Two tunnels are drilled in the bone through a trocar bushing. Threaded pins are driven in the tunnels, and a distraction hinge system is placed. It is followed with a gradual axial distraction of femur at 3-5 mm a day by tightening the threaded nail screws in various planes for two-three weeks. Thereafter, the apparatus is removed, and hip replacement follows.

EFFECT: method enables hip replacement with compensated shortening of a limb in high hip dislocation, soft tissues adaptation of the forthcoming surgery, provided anatomically correct position of the femoral head in relation to the cotyloid cavity, reduced postoperative complications.
