FIELD: medicine.

SUBSTANCE: preoperative design of the pelvic and hip implant components is performed. A line of resection of the greater trochanter is designed. Expanded access to joint is performed. Osteotomy of the greater trochanter in an oblique direction is performed from the inside out and top to bottom, as well as osteotomy of the neck and removal of the femoral head. Implant bed is prepared with mills and rasp. The pelvic and femoral prosthesis components are implanted. Head of prosthesis in the cavity is reduced. Bringing down the split-off trochanter is conducted in position of abduction and internal rotation of the hip to the location of its vertices in accordance with the rotation centre of the prosthesis. The slot within the split-off trochanter is modelled, which enables to impose trochanter on the femur and the upper-outer portion of the proximal leg of "press-fit" prosthesis implanted. Additionally, it is fixed to the femoral trochanter with three spongy screws. The wound is sutured in layers.

EFFECT: method enables to achieve an anatomically correct relationship of the greater trochanter relative to the implanted joint, secure fixation of the split-off fragments, reducing the time of treatment.
