SUBSTANCE: invention refers to medicine, namely to orthopaedics and traumatology. A joint is widely approached. A cotyloid cavity is located to start the treatment. After a cartilage and a subchondral plate have been removed from the bottom and walls of the cotyloid cavity within a defect zone of an end of the cotyloid cavity, a cortical bone is exposed. A V-pocket of the width equal to the width of the resected neck is formed perpendicularly to an articular surface of the cotyloid cavity. A head to neck transition is adjusted by a curvature form of the articular surface of the cotyloid cavity within the defect zone with removing the cartilage and the subchondral plate. The femoral neck is V-resected under the V-pocket with preserving the osseous structure as much as possible. Then a formed graft is tightly pressed to the prepared bed. A endoprosthesis bed is formed in the cavity and a portion of the head projecting therein.

EFFECT: method provides higher fixation strength of the bone graft, optimised conditions of the graft change.
