METHOD OF SURGICAL TREATMENT OF ESOPHAGEAL ACHALASIA Russian patent published in 2011 - IPC A61B17/00 

FIELD: medicine.

SUBSTANCE: invention relates to field of medicine and can be applied in surgery in case of laparoscopic elimination of esophageal achalasia. Essence of method lies in the following: longitudinal cut of muscular coat in lower third of esophagus and cardial part of stomach is made on anterior surface with preservation of integrity of their mucous membranes, which are later covered with fundus of stomach. After wide dissection of muscular coat from underlying layers of esophagus, left and right edges of dissected esophageal muscular coat are sewn with adjacent internal surfaces of left and right diaphragm pedicles in state of muscular diastasis in area of myotomic defect, which is made with width not less than half of esophagus wall circumference. After that, defect of muscular coat is covered with mobilised from His' angle transplant from fatty tissue on vascular pedicle, which is fixed, with probe introduced into esophagus lumen, in such a way, that posterior part of fundus was sewn with external surface of diaphragm left pedicle, anterior part of fundus being sewn with external surface of diaphragm right pedicle, and transplant from fattu tissue on vascular pedicle is located between fundus of stomach and myotomic cut.

EFFECT: application of claimed invention makes it possible to ensure prevention of achalasia recurrence and prevent development of reflux-esophagitis due to reliable sphincter-like action of anti-reflux cuff.
