METHOD OF SURGICAL MANAGEMENT OF PENIS DEFECT IN EPISPADIA AND EXTROPHY OF BLADDER Russian patent published in 2011 - IPC A61B17/00 

FIELD: medicine.

SUBSTANCE: skin is incised circularly along ventral and lateral surfaces of a coronary groove, a flap is cut out by bordering with an external urethral opening at a penis base and extending the incisions on a balanus up to its tip whereon tissue is incised longitudinally and sutured crosswise. Then, deformation of cavernous bodies is eliminated, separated on a dorsal surface, cleared of the base up to the tip. A chord is excised. A skin of an internal leaf of a preputial bag is used to cut out four free skin flaps: on the dorsal surface of both cavernous bodies, there are performed two N-incisions of a fibrous tunic. The borders of the fibrous tunic is mobilised within the incision by transforming the incision in square ones. The free skin flaps are moved to a defect zone of the fibrous tunic which is fixed to its borders by an uninterrupted blanket suture in the form of four autodermal inserts. A skin flap on the dorsal surfaces of the penis is formed in a tube on a urethral catheter inserted in a bladder. The formed neourethra is moved down between the separated cavernous bodies which are sutured together then by interrupted sutures over a neourethra. A wound is closed layer-by-layer on a trunk.

EFFECT: method is effective, provides high cosmetic and functional effect, a longer dorsal portion of the cavernous bodies and eliminated ventral deformation of the penis ensure its anatomically correct position with a vertical meatus, minimised number of postoperative complications.
