FIELD: medicine.

SUBSTANCE: group of inventions relates to surgery and can be applied for transduodenal papillosphyncterotomy. To fix major duodenal papilla used is device which consists of connected upper and lower internal rings with holes, connected upper and lower external rings with holes, fixing ring with three installed at angle legs, on whose apexes along 2 cm on external with respect to fixing ring side made are teeth, with upper internal ring transferring into left handle, upper external ring transferring into right handle, handles being fixed with rack; on lower internal ring on internal circumference radially at 120° angle from each other made are three projections with fixers; on external circumference of external ring radially at 90° angle from each other made are four eyes, with six thrusts being installed between upper internal and external rings, lower internal and external rings radially in centre-ward direction at equal distance from each other on circumference, in each of thrusts on free end placed are studs, two on the top sand two on the bottom, which are installed into holes of internal and external rings with possibility of travel in transverse direction of external rings, in hole of each thrust installed is blade, curved in plane in such a way that its convex part is directed in external direction, its lower part is lifted up, and in upper part of blade depression is made. Edges of cut are brought apart and device is applied with blades down. Handles of apparatus are brought apart, with blades opening duodenotomic opening. Device is sewn to intestine wall via eyes. Fixing ring is lowered into duodenotomic opening until fixation of major duodenal papilla occurs. When the operation is over, three legs are outpressed and fixing ring is removed from intestine lumen. Handles are brought together, with blades moving in reverse direction. Sutures are cut from eyes and device is removed from intestine wall.

EFFECT: group of inventions makes it possible to reduce trauma, improve fixation and visualisation of major duodenal papilla.
