FIELD: medicine.

SUBSTANCE: hip joint is surgically approached. An osteotomy of a neck is performed, and a head is removed. A cotyloid component of an endoprosthesis is placed into a true cotyloid cavity. A test hip component of the endoprosthesis is placed. That is followed by a femoral bone traction towards the cotyloid cavity to tense the muscles as much as possible, and the femoral bone is fixed in the achieved position. A distance L of the upper edge of the cotyloid cavity to the upper pole of the head of the hip component of the endoprosthesis is measured, and the latter is removed. A shortening osteotomy of the femoral bone below the lesser trochanter is performed at a distance along a longitudinal axis of the femoral bone of approximately 1.5-2 cm, and a bone fragment of a height equal to the pre-measured distance L is removed by the double angular dissection from the lateral side of the femoral bone. The hip component of the endoprosthesis is placed, and the endoprosthesis is assembled with a reposition of femoral bone fragments and lock-type reduction.

EFFECT: method enables minimising a risk of postoperative traction and neurotrophic complications and reducing the length of the rehabilitation period.
