FIELD: medicine.
SUBSTANCE: series of inventions relates to medicine. Compression-expansion device expansion of ramus of mandible consists of tubular housing. Inside the housing there is a lead screw, which on the left has section with left-hand thread and section with right-hand thread. On threaded sections of lead screw two carriages are placed. In the lower parts of carriages, protruding from the body, there are seats where two lugs linear shape are fixed by laser welding at an angle of 70° to carriage axis on both sides of the housing, having holes for bone screws. At the ends lead screw has left and right trunnions placed in sleeves and fixed by laser welding in housing ends. On the left trunnion protruding from the sleeve a washer is fixed by laser welding. Between the ends of sleeve and washer there is a gap. On the right trunnion protruding from the sleeve, first pivot coupling fork is fixed being part of the cardan shaft. Cardan shaft contains rods connected with second pivot coupling. Ends of one rod are connected to the first and second pivot couplings, and the second rod with one end is connected to the second pivot coupling and the other end is free with head suitable for wrench and has markings. Method of expanding necked ramus of mandible consists in the fact that angle and a branch of lower jaw are skeletonized from submandibular access, on the latter one mentioned compression-expansion device is applied so that its lugs are located parallel to longitudinal axis of branches, lead screw is output through an additional linear incision in the parotid region wherein lugs of the device are expanded at 2 mm and fixed with screws. Vertical middle ramus osteotomy is made with reciprocating saw and chisel, then lead screw creates compression on six days. Then the wound is closed in layers. Distraction starts on the seventh day and continues until ramus achieves width similarly to healthy side or to complete unwinding of device. Then upon completion of distraction is cardan rod shaft is broken in the area of cardan joint with the help of needle-holder, after this device remains completely under the skin. At the end of retention period regenerate conditions are assessed and in the presence of signs of bone regeneration device is removed.
EFFECT: invention provides expansion of ramus of mandible with minimum injuries, avoiding the need of additional operation of autograft abstraction, high degree of ramus of mandible expansion and reduced risk of relapse and inflammatory complications.
