FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to maxillofacial surgery and can be used for treating ankylosing injures of a temporomandibular joint (TMJ). Lower jaw is skeletonize in layers from submandibular access. Titanium surgical template made on the basis of a solid skull model constructed from computed tomography data of a specific patient is fixed on the jaw branches. Template is designed so that the curvature of the surface of the template corresponds to the curvature of the surface of the mandibular branch, and the rear edge of the template covers the posterior edge of the jaw branch. Template is followed by a horizontal osteotomy below a mandibular incision, followed by removing an ankylosing block within the TMJ, skeletonize of the skull with its treatment with a cutter under the joint socket. Glenoid fossa is installed on the base of the skull, with simulated cavities and projections corresponding to irregularities of the skull base of the specific patient, and fixed to bone by self-tapping screws, by means of four-five screws fixing to joint of lower jaw endoprosthesis of joint so that protrusion of endoprosthesis completely closed plane of osteotomy. Endoprosthesis head is inserted into the glenoid fossa.
EFFECT: method provides optimum functioning of lower jaw in treatment due to presence of artificial glenoid fossa of endoprosthesis and protrusion closing wound surfaces on base of skull and lower jaw.
