FIELD: medicine.

SUBSTANCE: correction of mechanical prosthesis dysfunction in the aortic position for children with CHD includes repeated installation of aortic valve (AV) prothesis. Prior to surgery, echocardiography is performed, peak and average systolic pressure gradients are determined at the valve prosthesis level, together with the effective indexed area of the valve prosthesis aperture, left ventricle contractile function. Repeated installation of the prothesis is performed at peak systolic pressure gradient of more than 50 mm Hg, average systolic pressure gradient of more than 30 mm Hh, area of the valve prosthesis aperture less than 0.85 cm2/m2 and left ventricular ejection fraction of 55-60%.

EFFECT: method allows prevention of intra- and postoperative complications due to the reduced rate of repeated procedures for mechanical prosthesis dysfunctions correction in the aortic position for children with CHD, by clarifying repeated installation indications based on the assessment of blood flow indicators in the prosthesis area, and justification of a possibility of a larger diameter prosthesis implantation for these patients.
