FIELD: medicine.
SUBSTANCE: crosslinking with the instillation of riboflavin with dextran is performed. During instillation of riboflavin with dextran, total anterior stromal puncture (ASP) is performed with oblique needle entry in the corneal stroma by a third of its thickness, using a sterile needle, prior to UV irradiation. Punctures are performed in parallel rows from sulcus to sulcus; two punctures are made to the lateral and medial sides at an angle of 45 degrees from one point in each row; the next bilateral puncture in the row is close to the previous one; punctures of the next row are performed in a similar way. Punctures in the direction of the previous row should reach the edge of the scars at their end, and the diverting punctures are performed in the direction of the unprocessed corneal surface, gradually covering the nasal and then the temporal half of the cornea. During puncturing, riboflavin with dextran is instilled every two or three minutes.
EFFECT: method provides relief of the pain syndrome and long absence of disease relapses as a result of a single procedure, and also promotes formation of a more closed scar surface, which makes scarring more closed, with fewer free windows in the cornea, compared to the scar barriers after the completion of total anterior stromal puncture according to the classical technique.
