FIELD: medicine.

SUBSTANCE: afferent blockade of six lower thoracic nerves (Th6 – Th12) and the first lumbar nerve L1 is performed by injecting a local anesthetic into the area of the square lumbar muscle interfascial space under ultrasound control. In this case, the puncture point for the administration of anesthetic is determined on the lateral surface of the abdominal wall at the midpoint of the distance between the costal arch and the iliac spine. The needle is advanced in the medial-lateral direction to the square lumbar muscle through the outer and inner oblique muscles of the abdomen, the transverse abdominal muscle, the chest-lumbar fascia. An anesthetic is then injected into the interfascial space between the fascia of the square lumbar muscle and the chest-lumbar fascia in the middle third of the posterior surface of the square muscle.

EFFECT: method allows to provide safe and effective analgesia in patients after surgery in the anterior abdominal wall and abdominal organs, including those having contraindications to epidural analgesia, reduces the probability of the retroperitoneal space and abdominal cavity structures damage.
