SUBSTANCE: invention can be used for surgical management of previously operated unclosed macular rupture. Three operative approaches located 4 mm from the limb are formed. A portion of the internal limiting membrane (ILM) of the retina around the macula which was not removed during the primary surgery is visualized. The remaining portion of the ILM is separated on the lateral side of the macula by means of vitreal forceps, forming a flap with a width of 1×2 mm, which is then separated from the rest of the layer. The tamponade of the macular rupture is performed by the separated portion of the ILM. The operation is terminated by an air endotamponade of the vitreal cavity. In the postoperative period the patient maintains the position face down within 1 day.

EFFECT: reducing the number of injuries accompanying the surgical intervention, reducing the patient's discomfort, eliminating the occurrence of relapses, closing the macular rupture, improving visual functions.
