METHOD FOR SURGICAL TREATMENT OF A FULL MACULAR HOLE, WHICH HAS CAUSED RHEGMATOGENOUS RETINA DETACHMENT Russian patent published in 2020 - IPC A61F9/08

Abstract RU 2715989 C2

FIELD: medicine.

SUBSTANCE: invention relates to medicine, specifically to ophthalmology. For surgical treatment of full macular hole (FMH), which caused rhegmatogenous retinal detachment after subtotal vitrectomy, liquid perfluororganic compound (PFOC) is introduced into the vitreous cavity to the level of vascular arcades, an internal limiting membrane (ILM) is separated on the side of the optic nerve disc by performing the first ILM capture outside the papillomacular beam at a distance of approximately 1 mm from the FMH edge, with subsequent stretching and placing the formed ILM valve into a macular hole. Then, liquid is replaced with air and a diathermic coagulation is used to perform a drain retinotomy from the temporal side. After draining the subretinal fluid, endolaser coagulation of the retina around the formed opening is performed. PFOC is replaced with air, wherein removal of PFOC from the vitreal cavity is performed from the temporal side. Operation is completed with the gas-air mix by tamponade of the vitreal cavity. In the postoperative period, the patient observes a downward position for 1 day.

EFFECT: method provides FMH closing, complete anatomical retina attachment, improved visual functions, reduced traumatism of the surgical intervention, no need to use vital dyes for visualizing ILM and performing an additional operation to remove silicone oil from the vitreal cavity, reduced risk of recurrence of retina detachment due to macular opening, reduced discomfort of patient associated with prolonged observance of forced position face downwards.
