FIELD: medicine.
SUBSTANCE: seated patient is inserted with a flexible fibroscope through the mouth into the esophagus. The operation is performed under local anesthesia, under visual and endoscopic control. The location of the future fistula is determined by the flexible fibroscope using transillumination. The movable end of the fibroscope is unfolded toward the anterior wall of the esophagus until it is contoured on the posterior wall of the trachea. The tracheoesophageal fistula is formed at the point of greatest endurance of the fibroscope end by a contact surgical laser. The fibroscope end is delivered to the tracheoesophageal opening. A conductor is led out through the fibroscope canal. The fibroscope is removed. A tracheoesophageal shunt is fixed to the adoral end of the conductor and set to the formed fistula.
EFFECT: possibility of an outpatient setting of the tracheoesophageal valve in patients after laryngectomy.
