METHOD FOR ASSESSING LIKELIHOOD OF PATIENT DEVELOPING VARIANTS OF TICK-BORNE INFECTIONS: NON-ERYTHEMATOUS FORM OF IXODID TICK-BORNE BORRELIOSIS OR COMBINED COURSE OF BORRELIOSIS-ENCEPHALITIS INFECTION Russian patent published in 2018 - IPC G01N33/53

Abstract RU 2647450 C1

FIELD: medicine.

SUBSTANCE: invention relates to medicine, namely to infectious diseases, and can be used in clinical practice to assess the likelihood of a patient developing variants of tick-borne infections: non-erythematous form of ixodid tick-borne borreliosis or a combined course of borreliosis-encephalitis infection. To do this, determine values of laboratory parameters of the patient in the acute period of the disease (within 10–14 days from the onset of the disease): proportion of CD4+, CD8+ lymphocytes, the absolute number of CD4+ lymphocytes, IgM levels, IL-8, gamma globulin, phagocytic index (FI), the number of phagocytic neutrophils (FN), Curdo index; determine the values of laboratory parameters of the patient in the period of convalescence (within 21–25 days from the onset of the disease): proportion of CD3+ lymphocytes, the absolute number of CD8+ lymphocytes, IgM level, the Curdo index. In the acute period of the disease, the values of the laboratory parameters of the patient in scores are estimated: the proportion of CD4+ lymphocytes at values ≤ 34.1 % – 4 points, proportion of CD8+ lymphocytes at values ≤ 24.01 % – 1 point, the absolute number of CD4+ lymphocytes at the values ≤ 678.07 mcl-1 – 3 points, IgM levels at values > 7.66 g/l – 6 points, IL-8 at values > 97.4 pg/ml – 6 points, gamma globulins at values ≤ 16.88 % – 4 points, parameters of FI at values > 35.6 % - 1 point, FN at values > 1,892.68 mcl-1 – 1 point, Curdo index with values ≤ -7.14 – 4 points; other values of indicators are estimated as 0 points; count the sum of points; at the sum > 11 points the probability of development of a combined course of a borreliosis-encephalitis infection is estimated equal to 82.25±0.06 %; for sum of ≤ 11 points estimate the probability of development of erysipelas-free tick-borne borreliosis of 84.81±0.05 %. In the period of convalescence, the values of the laboratory parameters of the patient of the acute period of the disease in points: the proportion of CD4+ lymphocytes at values ≤ 34.1 % – 4 points, proportion of CD8+ lymphocytes at values ≤ 24.01 % – 1 point, the absolute number of CD4+ lymphocytes at the values ≤ 678.07 mcl-1 – 3 points, IgM levels at values > 7.66 g/l – 6 points, IL-8 at values > 97.4 pg/ml – 6 points, gamma globulins at values ≤ 16.88 % – 4 points; FI at values > 35.6 % – 1 point, FN at values > 1,892.68 mcl-1 – 1 point, Curdo index with values ≤ -7.14 – 4 points; values of the laboratory parameters of the patient of the period of convalescence in points are estimated: the proportion of CD3+ lymphocytes at the values ≤ 74.11 % – 2 points, the absolute number of CD8+ lymphocytes at the values ≤ 480.45 mcl-1 – -1 point, level IgM at values > 1.59 g/l – 3 points, Curdo index with values ≤ -11.15 – 2 points; other values of indicators are estimated as 0 points. Count the sum of points; at the sum > 14 points the probability of development of a combined course of a borreliosis-encephalitis infection is estimated equal to 88.53±0.05 %; for sum of ≤ 14 points estimate the probability of development of erysipelas-free tick-borne borreliosis of 86.80±0.05 %.

EFFECT: use of this method allows to carry out differentiated diagnosis of no-erythematous form of ixodic tick-borne borreliosis or combined course of borreliosis-encephalitis infection.
