METHOD OF ADJUVANT RADIOTHERAPY FOR GLIOBLASTOMA Russian patent published in 2018 - IPC A61N5/10 A61B5/55 A61K49/06 A61P43/00

Abstract RU 2655095 C1

FIELD: medicine.

SUBSTANCE: invention relates to medicine, namely to radiation diagnostics and radiology, and can be used to conduct adjuvant radiotherapy for glioblastoma. Conduct an MRI of the brain with intravenous contrast enhancement. Detect the peripheral boundary of the accumulation zone of the contrast drug. Determine the boundaries of the irradiation target. Next radiation therapy is performed on the target area up to a SOD of 60 Gy. Series of native brain scanning programs complement the ASL-free perfusion. Determine the tissue perfusion in the accumulation zone of the contrast preparation. At a tissue perfusion value of 50–94 ml/100 g per minute, the absence of a residual tumor is diagnosed. At the same time target of irradiation is set 0.7–1.0 cm to the outside of the peripheral boundary of the accumulation zone of the contrast preparation. Irradiation of this target is carried out in the regime of the RPM of 2.5 Gy. With a tissue perfusion value of 95–180 ml/100 g per minute, a residual tumor is diagnosed. In this case target of irradiation is set 1.5–2.0 cm to the outside of the peripheral boundary of that part of the accumulation zone of the contrast agent, which is a residual tumor. Also target of the irradiation is set at 0.7–1.0 cm in the remaining parts of the given zone of accumulation of the contrast agent, which do not belong to the residual tumor. Irradiation of this target is carried out in the regime of the RPM of 3 Gy.

EFFECT: method ensures reduction in the radiation load on the intact brain substance and an increase in the radiotoxicity of adjuvant radiotherapy only in the zone of residual glioblastoma due to quantitative parameters of perfusion with subsequent selective final determination of the boundary of the irradiation target.
