System for the treatment or prevention of a pulmonary infection comprises a pharmaceutical composition comprising an aminoglycoside present in the liposome complex, as well as an atomiser that generates an aerosol of a pharmaceutical formulation at a rate of about 0.60 g per minute to about 0.80 g per minute. Composition is an aqueous dispersion, wherein the concentration of lipids is 45-60 mg/ml, the aerosol pharmaceutical composition contains a mixture of a free aminoglycoside and an aminoglycoside present in the liposome complex. Fine particle fraction (FPF) of the aerosol is about 64% or more, and the percentage of the aminoglycoside in the liposome complex after spraying is 60% to 70%. Also disclosed is the pharmaceutical composition and method of treating a pulmonary infection.
