FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to purulent and thoracic surgery, phthisiology, and can be used to treat pleural empyema. To do this, evacuation of purulent pleural contents and sanitation of the cavity with solutions of antiseptics are carried out. Then, after intrapleural photosensitization of the walls of the empyema with one of the medicines from the class of chlorins, phenothiazines or phthalocyanines, a bronchopleural fistula is identified by bronchoscopy in the staining in the region of the bronchus with fistula. In presence of staining, an endobronchial photodynamic therapy (PDT) is performed in the mouth of the bronchus with fistula. After this, a valvular bronchial blocking of the bronchus with fistula is performed. Then, after extraction of the bronchoscope, intrapleural laser PDT of the walls of the empyema cavity is performed with the multiplicity of intrapleural exposure 1-2 times a week from 3 to 10 procedures. Moreover, the PDT is carried out when observing the parameters of laser radiation at a wavelength of 660 nm for chlorins and phenothiazines or 675 nm for phthalocyanines at a dose density of 30–100 J/cm2.

EFFECT: method provides a local anti-inflammatory and antimicrobial effect regardless of the type of pathogen and spectrum of resistance to antibacterial drugs, destruction of the pyogenic layer in the walls of the empyema, reduction of the rates of purulent exudation during the transformation of purulent exudate into serous, early deposition of fibrin on the pleural sheets and their adhesion, a decrease in the rigidity of the collapsed lung, a decrease in the volume of the cavity of the pleural empyema.
