FIELD: medicine; thoracic surgery.

SUBSTANCE: by thoracoscopy, the degree of severity of the inflammatory process of the pleural cavity is assessed. Localization of the mouth of bronchopleural fistulas is revealed. Sanitation of the destruction cavity is carried out using a vacuum system. In the course of surgical access — mini-thoracostomies carry out occlusion of the bronchopleural fistula from the side of its mouth by positioning on the visceral pleura in the area of the xenopericardial plate lung defect. The xenopericardial plate is sutured with a serous surface to a finely porous sponge of the vacuum system. A vacuum aspiration drainage system is installed. Vacuum aspiration of purulent-destructive discharge from the destruction cavity is carried out under negative pressure.

EFFECT: method allows to increase the efficiency of treatment of pleural empyema with bronchopleural fistulas, regardless of the diameter of the bronchopleural fistula and the severity of changes in the pleural cavity, to reduce the duration of inpatient treatment, and to reduce pain during treatment.
