SUBSTANCE: invention relates to medicine, namely to surgical hepatology, and can be used to assess the biosynthetic function of the residual liver parenchyma when performing extensive resection. Based on the results of ultrasound, the volume of the liver parenchyma is determined at the stage of preoperative planning. Volume of the resected part of the liver and the residual liver parenchyma, serum albumin is determined. Calculate the liver resection index (IRP) by the formula: IRP = Vres/Sa x vres/Vhep = Vres2/SA x Vhep (ED), where Vres – the volume of residual liver parenchyma (cm3); Vhep – liver volume (cm3); SA (serum albumin) – serum albumin (g/dl). With an IRP value of less than 15 IU, the risk of developing life-threatening postoperative complications is high. When the value of the IRP from 15 to 20 IU – the risk of developing moderate postoperative complications is high. If the value of the IRP is more than 20 IU, the prognosis for surgical treatment is favorable.

EFFECT: method allows to reduce the incidence of postoperative liver failure after performing extensive resection by assessing the biosynthetic function of the residual liver parenchyma when planning the volume of surgical intervention.
