FIELD: medicine.

SUBSTANCE: invention relates to abdominal surgery. Performing upper-median laparotomy with widening of access to the right. Abdominal organs are inspected. Right lobe of liver is mobilized by transection of triangular, coronal and falciform ligaments. Elements of the hepatoduodenal ligament are mobilized: a right branch of the portal vein is isolated along the entire circumference, with obligatory differentiation, visualization and isolation of a vein of the fourth segment of the liver, as well as a right hepatic artery. Right branch of the portal vein is ligated in the proximal direction from the origin of the vein of the fourth segment with separate ligation of the latter without crossing them. Right branch of the hepatic artery is ligated. Right hepatic vein is isolated along the entire circumference within the caval gate of the liver, ligated by applying a polymer suture without crossing them.

EFFECT: method makes it possible to perform portal-caval separation of blood flow of the right lobe of liver, to minimize the risk of developing postresection hepatic failure, to reduce the risk of distant metastasis, extending the allowable waiting time for the formation of vicarious hypertrophy in cases when liver growth is slowed down in fatty liver disease, the presence of chemotherapy consequences, cirrhotic organ parenchyma.
