METHOD OF SURGICAL TREATMENT FOR PATIENTS WITH ESOPHAGEAL CANCER Russian patent published in 2018 - IPC A61B17/00

Abstract RU 2673101 C1

FIELD: medicine.

SUBSTANCE: invention relates to medicine, namely to surgery, and may be applicable for surgical treatment of esophageal cancer. Cervical lymphadenectomy is carried out on the affected side, retaining all the neurovascular elements of the larynx. Recurrent nerve is isolated and the lobe of the thyroid gland is removed from the side of the tumor, providing access to the side wall of the esophagus. Larynx is rotated in the opposite direction. Esophagus is separated from prevertebral fascia. Tumor is visualized, determining resection boundaries. Esophagus is removed on the turnstile into the wound. Crossing trachea at the level of 3-4 semirings and recurrent nerve. Resection of the cervical esophagus is performed possibly beyond tumor borders. Restoring continuity of the food pathway with a segment of intestine on anastomoses and revascularize it through neck vessels, while segment of the intestine is sutured proximally to pharynx, and in distal direction to the resected esophagus. Stitches between tracheal rings along posterior and lateral walls, and recurrent nerve is stitched end-to-end. Larynx is set in the usual position. Preventive tracheostomy is formed, neck wound is drained and sutured.

EFFECT: method allows to reduce trauma, provide vocal, respiratory and separation functions of the larynx.
