FIELD: medicine.
SUBSTANCE: invention relates to the field of medicine, namely to traumatology and orthopedics during surgical treatment for closed internal fixation in case of traumatic fractures of the anterior part of the pelvic half-ring. Device for placement and fixation of a metal implant and fragments of bones of the anterior part of the pelvic half-ring contains a jig in the form of a metal tube, made with the possibility of accommodating a removable sleeve, and a means of fixing the metal implant. Parallel to the conductor, a tubular guide is installed, one-piece fixed at one end on a holder into which said conductor tube is inserted and screwed by means of a lining. Inner diameter of the guide and conductor tubes exceeds the size of the self-tapping head of the means for fixing the metal implant to the bone. Device is equipped with at least one removable sleeve, the outer diameter of which does not exceed the inner diameters of the guide tube and the conductor tube. In this removable sleeve the through axial hole is made for the drill or for the installation needles with the ability to ensure their sliding movement in it. Said means for fixing the metal implant is made in the form of a collet clamp that includes longitudinal collet legs formed by longitudinal section of the conductor tube and carrying at their ends a U-shaped grip forked by a longitudinal section that is attached to the cylindrical part of the legs, which is a continuation of the conductor tube, where movable tubular collet lock is mounted with the possibility of free sliding. Metal implant is made in the form of a fixing plate having two oval holes along its axis with a center distance equal to the distance between the axes of the guide tube and the conductor tube, and two blind holes across the specified axis of the plate, mated in the closed state of the collet clamp with the corresponding protrusions on the U-shaped grip. These legs have longitudinal ledges associated with a fixing plate, and means for extending the legs of a U-shaped grip, made in the form of a weakened section of each foot at the place of their bend by means of a transverse curvilinear profile of the groove on its outer surface and side bevels with through holes, made with the possibility of angular movement of the tabs from the closed position at an angle α, which ensures the disengagement of the projections of the U-shaped grip with the holes on the fixing plate. Inside the legs in their closed state, in the zone of the U-shaped grip, an annular groove is made, in which an elastic element in the form of a split ring is placed with the possibility of elastic movement of the legs by an angle exceeding the specified angle α, and the ends of the other end of the guide tube and the end of the conductor tube are cut by a transverse inclined plane at an angle of 45° to the corresponding axis. Specified inclined plane of the guide tube coincides with the inner surface of the U-shaped gripper collet clamp, made with the possibility of free axial movement of the head of the self-tapping screw inside the gripper. Collet lock is provided with a spring-loaded ball mated when the metal implant is fixed with a U-shaped gripper with a circular groove on the conductor tube. One of the parts of the U-shaped grip is equipped with a bone stopper, made with the possibility of ensuring the working position of the conductor relative to the upper edge of the anterior part of the pelvic half-ring.
EFFECT: invention provides the ability to restore the anatomical shape of the anterior part of the pelvic half-ring by securely fixing the implant in a predetermined position.
