SUBSTANCE: invention relates to otolaryngology. Stirrup is mobilized by removing foci of tympanosclerosis. Ossicular prosthesis is mounted. With the safety of the stirrup a partial prosthesis is mounted, and in the absence of the stirrup a full prosthesis is mounted. Closure of the defect fascial autograft is carried out. Additionally, an auto-bone graft of a cylindrical shape with the length of 4–5 mm and diameter of 0.6–1.0 mm is formed intra-operatively, which is placed on a fascial autograft covering the defect, while simulating the location of the natural handle of the malleus. It is fully covered with a second fascial autograft with the diameter of 5–6 mm.

EFFECT: method allows to effectively implement tympanoplasty with an open form of tympanosclerosis when fixing the stirrup in the absence of the handle of the malleus and the long leg of the incus, to exclude the first stage of the subsequent two-stage surgical treatment due to prophylactic creation of a support for the prosthesis.
