FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to oncology, operative gynecology. Conglomerate consisting of tumors of ovaries, uterus, uterine ligaments, peritoneum of pelvis and parametric cellulose is mobilized by retroperitoneal approach and removed in a single unit with an upper third vagina. Round, cardinal and sacrouterine ligaments cross at the pelvic wall without affecting the lower hypogastric plexus (plexus hypogastricus inferior). Biliary-uterine ligaments are transected near bladder wall. Uterine artery and vein are transected at the level of their retraction from the internal iliac artery and vein. Pelvic and para-aortic lymph nodes are completely removed in a single unit – total paraaortal and iliac lymph node dissection.
EFFECT: method enables achieving 5-year overall survival rate in 96 %±3,9 % and 5-year recurrence-free survival in 76,1 %±11,3 % of patients with advanced ovarian cancer.
