METHOD FOR INTRA-SINUS ELIMINATION OF MAXILLARY SINUS UPPER WALL DEFECTS AND DEFORMATIONS AND ENOPHTHALMOS CORRECTION Russian patent published in 2019 - IPC A61F9/07

Abstract RU 2690906 C1

FIELD: medicine.

SUBSTANCE: invention refers to maxillofacial surgery, ophthalmology, otorhinolaryngology, and can be used for intra-sinus elimination of maxillary sinus defects and deformations and enophthalmos correction. Maxillofacial gingival mucosa is incised, and an anterymous sinus wall is skeletonised. Titanium plate is placed. Titanium plate has a horizontal portion, a distal portion bent at angle of 90° down, and a proximal portion bent at angle of 90°. Distal and proximal parts have through holes. Computer 3D tomography is used to diagnose the exact position of the bone defect of the upper wall of the maxillary sinus. Titanium plate is modelled congruently to the defect or deformation so that its horizontal part adjacent to the defect exceeds its dimensions along perimeter by 1 mm. Bone window is formed in the area of an anterior sine wall by means of a cutter. Schneider membrane is mobilized from an upper wall of the maxillary sinus. Bone fragments of maxillary sinus upper wall are repositioned. Distal portion of titanium plate is inserted into maxillary sinus. Pins are inserted through the through holes and the plate is fixed to the back mandarin wall of the maxillary sinus. Horizontal part of the titanium plate compensates the defect of the upper wall of the maxillary sinus. Proximal portion of the titanium plate is fixed through the through holes to the lower-vertex edge by means of pins. Schneider membrane is lifted upwards and fixed to upper wall of maxillary sinus by fibrin glue.

EFFECT: method enables surgical intervention with minimal traumatism, avoiding facial incisions, improving the therapeutic outcome, including cosmetic treatment, performing the enophthalmos correction, reducing the operation time and injuries by using the optimum design of the titanium plate.
