FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to surgical dentistry and otorhinolaryngology, and can be used for intrasinal implantation to eliminate maxilla adentia. 3D computed tomography, a maxilla condition assessment, a structural simulation, and a mucoperiosteal flap are cut out. Structure is installed. Structure consists of three parts: L-shaped part, vertical cylindrical part and abutment. L-shaped part consists of a vertical flat part and a horizontal cylindrical part. Notch is formed in alveolar process of upper jaw of cylindrical shape, width 4 mm. Window is formed along the anterior wall of the maxillary sinus with size of 10 by 5 mm. Further, Schneider's membrane is mobilized upward and inside the sinus to expose the inner surface of the zygomatic bone and the outer wall of the maxillary sinus. L-shaped part of the structure is introduced into the cavity of the maxillary sinus through the formed window, the vertical flat part of which is fixed through the holes with screws by congruent to the internal wall of the zygomatic bone. Then vertical cylindrical part is introduced into notch in alveolar process of upper jaw. Horizontal cylindrical part of L-shaped part of structure and vertical cylindrical part are connected. Blind and through holes are aligned and fixed with a screw. Below to the vertical cylindrical part on the threaded rod the abutment is fixed, on which the orthopedic structure is further fixed.
EFFECT: method provides reliable primary fixation of structure, adaptation of structure to any part of alveolar process within maxillary sinus, reduced length of time of rehabilitation and prosthetic repair, possibility of one-stage prosthesis, replacement and implantation in case of considerable defects of alveolar crest due to use of the proposed structure.
