METHOD FOR RESTORING BACKBONE COMPLEX INTEGRITY IN RESECTED LAMINECTOMY Russian patent published in 2019 - IPC A61B17/00

Abstract RU 2699724 C1

FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to traumatology and spinal surgery, and aims at restoring integrity of a posterior support spine complex in a resected laminectomy. In patient's post lying on abdomen under endotracheal anesthesia, posterior median incision is made in projection of spinous processes of required level. Skeletonisation of spinous and articular processes, vertebral arches at this level. Posterior rigid transpedicular fixation is performed under a fluoroscopic / CT control; pedicle screws are introduced bilaterally into the legs of the vertebrae. Core is contoured by bending of lordosis of fixed segments. TPF system is mounted. Plastic structure, namely a synthetic resorbable material is applied to prevent / prevent the adhesive process on the contents of the spinal canal. Titanium mesh modeled according to the spinal canal is applied on the bone laminectomucleus defect and placed under the pedicle screw heads. Titanium mesh used is titanium silk material, which is pre-cut with exceeding the size of the defect from the medial side by 3–4 cm, and on the other three sides – by 1–1.5 cm. Medial side of the implant is placed over the spinous process with margin at 1–1.5 cm behind the counter-side of the process, fixing the implant with a suturing suture along the supragingric ligament. From the lateral side of the implant in the area of the screws, the implant is grasped by ligatures and fixed under the screw heads; bone substitute material is placed over the implant.

EFFECT: method by reducing traumatism and providing prevention of cicatrical and adhesive processes enables higher effectiveness of surgical intervention in recovering the integrity of the posterior support spinal complex.
