SUBSTANCE: invention refers to medicine, namely to reconstructive surgery and traumatology. Bone fragments of anterior shin bone and second metatarsal bone are performed on a single vascular pedicle by mobilizing an anterior tibial beam to its intersection in the upper one-third of the shin with muscular branches of the deep fibular nerve. Then an anterior tibial muscle is withdrawn more proximally. Muscular branches of the deep fibular nerve are visualized and isolated to the anterior tibial muscle, monitoring the integrity of the recovered muscular branches of the deep fibular nerve, an anterior tibial vascular bundle is separated to an upperlateral fibular artery. Formed complex of tissues is performed in proximal direction under selected muscular branches of deep fibular nerve. Tunnel with a hip opening in the reconstruction area is formed through a shin wound in the subcutaneous fat between the projections of tuberculum Gerdy and the head of the fibular bone. Complex of tissues is delivered through the formed tunnel with feeding in the upper one-third of the shin of the feeding vessels along the arc on the surface of the tendon of the anterior tibial muscle. In the area of a false joint, forming a bed for a bone fragment of the second instep bone in another sector of the femoral bone relative to the bone fragment bone of the tibia. Vascular fascicle between the bone fragments is looped. Then, the bone autografts are fixed.

EFFECT: invention provides higher osteogenesis in femoral reconstruction, reduced traumatism, reduced risk of postoperative complications, reduction of treatment time due to features of extraction and formation of a good blood-supplying bone autograft with its further laying in a hip defect area through a specially formed tunnel.
