SUBSTANCE: invention refers to medicine, namely to reconstructive microsurgery. An autograft contains a single complex of a tibial periosteum, a tibial coritical plate, a fragment of extensor hallicus longus, an anterior tibial artery and comitant veins. In a projection of an intermuscular sulcus of an anterior group of shin muscles, an anterior tibial neurovascular fascicle is separated with mobilisation and abduction towards a deep fibular nerve. The fragment of extensor hallicus longus in the form of a muscular sleeve, septal vascular branches from the anterior tibial vascular fascicle, and perforating branches through the fragment of extensor hallicus longus, the tibial periosteum including greater periostal vascular branches are separated on vascular associations from the anterior tibial vascular fascicle in a single complex. Then the periosteum is dissected with forming a section of required size. The tibial cortical plate is perforated; the anterior tibial vascular fascicle is ligatured and transected in the distal direction from the formed complex of tissues. The coritical plate is separated by perforation lines, and the autograft is taken.

EFFECT: group of inventions provides reception of breakthrough complex of tissues with repair and osteogenic properties, enabled closure of greater defects of recipient areas, decreased postoperative complications, reduced length of treatment.
