METHOD FOR CAPITELLUM RECONSTRUCTION IN CASE OF ASEPTIC NECROSIS Russian patent published in 2017 - IPC A61B17/00

SUBSTANCE: deep artery of the shoulder and the collateral radial artery with the comitant veins are mobilized from the radial nerve and surrounding tissues, from the exit from the spiral canal to the level of the distal metadiaphysis of the humerus. In a single complex with the isolated vessels, their branches and branches of the recurrent radial artery to the periosteum of the external epicondyle of the humerus, a periosteal cortical autograft is raised. The periosteum is raised in the form of flaps from the anterior and posterior surfaces of the external epicondyle at the level of attachment of the long radius hand extensor, including its fragment. After that, periosteal flaps are turned laterally, the cortical plate along the perimeter of the taken autograft is perforated and lifted, the cortical plate in the capitellum is perforated proximal to the fracture level, forming a bed in the spongy substance of the metaphyseal part. Then, in the subcutaneous tunnel, the tissue complex is rotated proximally, the cortical part of the autograft is placed in the formed bed, the periosteal flaps are spread sideways over the capitellum surface and fixed.

EFFECT: method allows to increase the effectiveness of surgical treatment and reduce treatment time due to the full implementation of the potential possibilities of the transplant.
