METHOD FOR SURGICAL TREATMENT OF URETER LONG STRICTURES Russian patent published in 2020 - IPC A61B17/00

Abstract RU 2712066 C1

FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to urology and surgery. Method consists in cutting a flap of size 6×3 cm on the anterior surface of the urinary bladder wall. Antireflux mechanism of anastomosis is created. Tunnel is formed for up to 3 cm in a mucosal layer of the cut out flap. Mucous flap is dissected throughout the total length, which is formed from the length of the section of the spilled ureter and the length of the submucosal tunnel. Edges of dissected mucosa in the lateral directions are prepared by 5 mm. Spilled ureter is placed into formed bed. In the distal incision region, the edges of the incised mucosal graft are sutured together with the edges of the mated spleen of ureter 5-6 sutures along the entire circumference with non-traumatic biodegradable suture material. Ureter wall is inserted into a dissected submucous tunnel and fixed to the mucosa of the flap. Fixation of the ureter is carried out either by suturing the dissected mucosa edges over the ureter in the case of its diameter matching to the size of the formed bed, or by directly fixing to walls of formed bed with maximum possible ureteral coating edges of dissected mucosa in case of exceeding ureter diameter of formed bed size. Stent is inserted into the ureter and a tubular structure is formed from the flap by folding it. Tubular structure is sutured with a continuous atraumatic monocryl suture.

EFFECT: method enables providing more effective surgical treatment of extended ureter strictures by preserving the integrity of the tunnel mucosa and maintaining the constrictive mechanism preventing vesicoureteral reflux.
