SUBSTANCE: invention refers to medicine, namely to vascular surgery, and can be used for combined treatment of chronic arterial insufficiency of lower extremities of III-IV degree. That is ensured by revascularising laser osteoperforation of soft tissues and bones of the affected extremity, and administering angiotropic preparations in the pre- and postoperative period. Regardless of the presence and nature of necroses, all hepatitis 7–10 days after laser osteoperforation is administered with heparin 10,000 units/day - 2,500 units. 4 times a day subcutaneously under the control of blood coagulation.

EFFECT: method provides higher clinical effectiveness by improving the prediction of the success of foot-preserving foot surgeries or performing amputations of extremities not above the shin level - maximum possible distalization of amputations, while reducing the time of revascularization effect ensured by eliminating the negative microcirculatory effect of laser osteoperforation, the arteriovenous shunts in the upper and middle thirds, the progression of necrotic tissue changes.
