FIELD: medicine.
SUBSTANCE: method of access to structures of various spine parts involves making a cut, conducting a guide wire, sequential installation of the cannulated wound dilators, measurement of wound depth, installation of a retractor of a certain depth and its fixation. After making the incision and installing the guide wire, the following device is used, wherein in the guide wire is introduced until contact with the bone structures of the spinal column, using a central through channel, small, then large round cannulated dilators. Then, an oval dilator of the above device is installed, using a large round dilator as a guide. Depth of the surgical wound is determined on the scale located on the outer surface of the dilator. Using an oval dilator as a guide, a corresponding tubular retractor is inserted into the wound. Bracket is used to fix retractor to operating table and perform fluoroscopic inspection. Then, the wire and dilators are removed from the wound to form a single oval working channel inside the retractor. Device for access to various structures of spinal structures consists of a tubular retractor with an oval tube and a bracket for attachment to the operating table, small and large round cannulated dilators, guide wire for a small round cannulated dilator of an oval cannulated dilator, on outer surface of dilators there are scales. Small and large round dilators are made in the form of tubes of round cross-section with chamfers on the distal end. Oval cannulated dilator has an oval outer section for guiding the tubular retractor, chamfers on the end and with a round hole for the large round cannulated dilator diameter along the central axis.
EFFECT: inventions provide the implementation of the planned minimally invasive spinal intervention, increasing the surgical area with the need to improve the view and expansion of access with minimal traumatisation of surrounding tissues, reduced time of surgical intervention, reduced recovery period.
