FIELD: medicine; ophthalmology.

SUBSTANCE: invention can be used to prevent vitreous prolapse during penetrating antiglaucoma surgery in patients with incompetent lens ligaments. A conjunctival incision is made. A superficial scleral flap is formed at 1/3 of the depth of the sclera with the base towards the limbus. A deep scleral flap is cut out without opening the anterior chamber. Using a cannula, a viscoelastic drug is injected under the iris through the formed corneal paracentesis in the projection of the planned coloboma and fistula. The deep scleral flap is removed and the anterior chamber is exposed. An iris coloboma is formed, through the said iris coloboma a part of the viscoelastic is evacuated, and the vitreous body is kept in the vitreal cavity. The superficial scleral flap is placed in the scleral bed. Sutures are placed on the superficial scleral flap and conjunctiva. Paracentesis is hydrated.

EFFECT: prevention of vitreous prolapse through iris coloboma and trabecular fistula, which in turn prevents blocking of the formed outflow pathways of intraocular fluid and thereby ensures a long-term hypotensive result of antiglaucoma surgery.
