FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to cardiology, and concerns selection of patients with decompensated ischemic chronic cardiac failure (CCF) for endomyocardial biopsy procedure. That is ensured by clinical and instrumental examination of the patient and selection is carried out in the presence of the following complex of criteria: dyspnea in physical activity/rest, orthopnea, dry cough, peripheral edema, heart failure, heart pain, persistent elevation of myocardial necrosis markers (troponin I, creatine kinase, creatine kinase MB), presence of local myocardial contractility disturbance, which does not correspond to the coronary artery basin, which caused ischemic myocardial injury, as well as possible violation of basal and apical rotation and systolic velocities according to echocardiography with 2D-speckle tracking technology.
EFFECT: detection of specified criteria provides high accuracy of diagnosing latent myocarditis, which is the cause of decompensation of ischemic CCF with systolic dysfunction LV and/or continuing unfavorable cardiac remodeling following the optimal myocardial revascularization.
