FACIAL NERVE END-TO-END PLASTY METHOD WITH ITS INTRATEMPORAL INJURIES Russian patent published in 2020 - IPC A61B17/00 

FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to otorhinolaryngology and can be used in rehabilitation of patients with facial nerve intratemporal injuries. It involves an expanded antromastoidectomy with removing an apex of the mastoid process and a posterior wall of the external auditory canal. Facial nerve stroke is identified in its bone canal. Injured segment of the facial nerve is excised within the unchanged tissues. That is followed by complete decanalization of the facial nerve from the ganglion geniculi to the stylomastoid foramen. Then the nerve is released from the case in a stylomastoid foramen; a cervical branch is separated and incised. Then, the resected ends of the facial nerve are brought together and an end-to-end neurorrhaphy is performed. Epi-perineural suture is applied. Nerve trunk is covered with autofascia. Further, autocartilage plates are placed on a horizontal semicircular canal. Ossiculoplasty, myringoplasty, meatoplasty and loose tamponade of tympanic and mastoid sections are performed. Surgical wound is closed in layers.

EFFECT: method enables nerve plasty in confined space of middle ear cavity and creates optimal conditions for its further recovery.
