FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to urology and oncourology. Patient is simultaneously removed from the prostate gland and the areas of edenopopvial and peritoneal-perineal fascia. First, a patient with an anterior abdominal wall is prepared with a three-layer graft consisting of a parietal peritoneum, a preperitoneal tissue and a transverse fascia with a width of at least 4 cm and a length of not less than 10 cm, after which the graft fascial side is laid in the transverse direction on the rectal anterior wall into the peritoneal-perineal fascia defect zone, and additionally the edges of the latter are sutured with an absorbable suture with the corresponding edges of the three-layer graft. Further, urethrocystoneoanastomosis is formed, then free ends of three-layer graft are brought downward front wall of urethrovesical anastomosis and sutured together with absorbable suture, thus closing fascial ring above urethrocystoneoanastomosis zone. Thereafter, the front semi-circle of the graft is fixed with separate absorbable sutures to puboprostatic ligaments and a bladder wall.
EFFECT: method enables the most complete recovery of the fascial anatomy of the small pelvis after the RPE stage with preserving the preoperative spatial and functional interrelation of the pelvic organs, allowing complete stabilization of posterior and anterior parts of urethrocystoneoanastomosis with absolutely any volume of removed prostate, which leads to early and complete restoration of urine retention function in operated patients, and also enables to minimize risk of failure of UVA or formation of urethral stricture, thereby improving effectiveness of surgical intervention.
