METHOD OF FORMING SPHINCTER-SPARING URETHROVESICAL ANASTOMOSIS Russian patent published in 2024 - IPC A61B17/00

FIELD: medicine; operative urology and oncology.

SUBSTANCE: when performing a radical prostatectomy, the urethrovesical anastomosis is formed with two continuous threads with notches 23 cm long on a 5/8 needle each, starting from the posterior reconstruction; the first thread is used to suture the remains of Denonvilliers' fascia with the transected urethrorectal muscle and the median perineal suture. Next, using the same thread from 6 o'clock position on the conventional dial, moving clockwise, an anastomosis is formed between the mucous membrane of the bladder and the membranous mucosa of the urethra until reaching 10 o'clock on the conventional dial. Then, with the second thread, starting from 6 o'clock position on the conventional dial, moving in the opposite direction, form the left semicircle of the anastomosis until reaching 2 o'clock position on the conventional dial. Next, the anterior semicircle of the membranous part of the urethra is immersed into the lumen of the bladder, forming sutures between the wall of the bladder and the crossed dorsal venous complex, in the interval between 10 and 2 o'clock position on the conventional dial. Then the endopelvic fascia is restored on both sides along its entire length by suturing the bladder detrusor to the puboprostatic ligaments and the parietal layer of the intrapelvic fascia.

EFFECT: method ensures maximum stabilization of the pubourethral complex in the pelvic cavity with the aim of full and early return of continentity.
