FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to surgery and oncology. Avascular loop of small intestine 30 cm long from the Tretiz's ligament is passed through the avascular window in the mesentery of the transverse colon. Between posterior wall of oesophagus stump and leading loop of intestine continuous blanket seromuscuscular-submucosal sutures are applied in two rows, lateral threads after tying with free thread passed through last stitch of blanket suture are taken on holders. Then, in compliance with oesophageal diameter, gut wall is dissected transversally and oesophagus is anastomosed to adducted intestine side with canted by serous-muscular-submucosal suture. Front row of internal sutures of an anastomosis is immersed into a leading intestine, alternatively by means of four semipurse-string sutures - by means of distal and proximal lateral sutures taken earlier on holders on both sides, and three "sliding" sutures between the distal half-filament sutures, the transverse shoulder of which passes through the oesophagus wall with the submucosal layer. "Sliding" sutures are fastened, the wall of the adductus is wrapped, the oesophagus is invaginated. Then at distance of 12 cm from the anastomosis, a leading loop of the small intestine is sutured crosswise with a mechanical two-row seam, which is covered with a viscous serous-muscular-submucosal suture, mesenteric edges of the adducting and abducting intestinal loops are sutured together by the same suture higher by 5.0 cm and less by 3.0 cm from the mechanical suture. There are formed two intestinal aminostasis according to Brown, - proximal anastomosis for food passage from the intestinal loop with length of 4.0 cm and a distal length of 1.5 cm for passage of bile and pancreatic juice.
EFFECT: method prevents entero-oesophageal reflux, reflux oesophagitis, peptic complications on the side of oesophagus and anastomosis, provides more physiological fractional food intake.
