SUBSTANCE: invention refers to medicine, namely to oncology, and can be used for treating rectal cancer. Method involves administration of capecitabine in dose of 1650 mg/m² orally 2 times a day on days of radiation therapy, there is no reception on weekends, followed by 6 courses of consolidating polychemotherapy in the form of SarOx: oxaliplatin 130 mg/m² per day 1 and capecitabine orally in dose of 2000 mg/m²/day, which is divided into two daily doses for 14 days, then a seven-day pause. Remote radiation therapy is carried out 5 days week from 1 to 5 days in dose of 2Gy to a total basic dose of 52–58 Gy. If the initial tumor size is reduced by more than 50 %, local excision of the residual tumor is performed using the transanal microsurgery technique.

EFFECT: using the invention enables complete therapeutic tumor pathomorphosis in 100 % of patients, reducing the rate of toxic and postoperative complications as a result of absence of preparation on weekend days and increased interval between end of preoperative therapy and surgical intervention up to 8 weeks, increasing organ-preserving surgical interventions to 100 %.
