SUBSTANCE: invention refers to medicine, namely to oncology, and concerns a method of treating rectal cancer. Method of treating the patients with rectal cancer using a personalized approach consists in the fact that the patient undergoes a chemotherapy according to CAPOX 4 course regimen followed by the pelvic MRI examination to assess the effect. If the tumor regresses or stays stable for 2 weeks, operation and 4 courses of the CAPOX chemotherapy are performed. In case of tumor progression with underlying chemotherapy, chemoradiotherapy is presented with SBD 2 Gy, TBD 50 Gy with chemotherapy of capecitabine 825 mg/m² 2 times a day on the days of radiation therapy. That is followed by 8-12 weeks of surgery and 2 courses of the CAPOX therapy.

EFFECT: personalized approach to treatment of patients with rectal cancer, including those resistant to chemotherapy, enables to achieve a complete chemotherapy course in patients with rectal cancer, achieving complete morphological response to treatment of patients, achieving therapeutic pathomorphosis of patients, in whom progression of rectal cancer with chemotherapy background was observed.
