FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely traumatology and orthopedics, and may be used for operative treatment of displaced acetabular fractures. The patient is laid down on the side, external-anterior access to the hip joint is performed. Anterior dislocation of the femur is performed as well as osteotomy of the neck of the femur. The femoral head is removed. The trochanteric part of the thigh is retracted posteriorly. The displaced acetabular columns are brought closer to each other, achieving reduction. Then the anterior and posterior columns of the cavity are fixed with wires, a repositioning-fixating support ring is installed in the acetabulum, having the shape of an open hemisphere and a contour corresponding with the contour of the cartilaginous surface of the acetabulum. The support ring contains two ends, whereon it has one hole and one notch for insertion of fastening screws through them into the acetabulum, the inner and outer surfaces, the upper inlet and the lower opening, an array of holes and notches along the edge of the wall along the entire perimeter of the ring, intended for passing the fastening screws through them. The ring is fixed to the articular surface of the main fragment of the acetabulum connected to the ilium by inserting screws through the holes of the support ring. Then a sequence of screws is inserted at an angle into the posterior column through the corresponding hole of the ring and its articular surface, and into the anterior column through the corresponding hole of the ring and its articular surface, leaving a part of each screw above the inner surface of the ring. The wires are removed from the columns. The reposition of the acetabulum is continued by tightening the columns by the screws and creating mutual simultaneous compression between the main fragment of the acetabulum connected with the ilium, the posterior and anterior acetabular columns, on the one hand, and the outer surface of the ring, on the other side. The screws are alternately screwed into the rear and the frontal columns until the screw head stops at the edges of the ring hole. Load is applied to the pelvic bones for possible mobility between the columns and between the columns and the ring. Screws are inserted into the acetabular columns through every free hole of the ring to strengthen the fixation of the columns to the ring.
EFFECT: method provides a reduction in the cost of treatment, early rehabilitation of the patient and early recovery, active recovery of movement in the operated joint due to using of only one external-anterior access to the hip joint and gradual reduction.
