FIELD: medicine; traumatology; orthopedics.
SUBSTANCE: invention relates to medicine, namely to traumatology and orthopedics, it can be used as a method of open low-traumatic reposition of pillars and fragments of the acetabulum in an experiment. On a model of the pelvic bone, a type B2.2 acetabular fracture according to the international classification AO/ASIF is modeled by osteotomy of the acetabular cavity with the creation of diastasis between the pillars. At the next stage, the displaced pillars are brought closer to each other, achieving the most accurate reposition. Next, a repositioning-fixing support ring of the appropriate size for the acetabular cavity is installed in the acetabular cavity and fixed with fixing screws to the main fragment of the acetabular cavity connected to the iliac bone. After the achieved reposition, a through channel is drilled through the joint surface and the entire thickness of the bone wall of the posterior pillar. In the drilled channel, guide tubes with threaded wires are inserted so that their ends stand out from the drilled channel from the side of the inner surface of the sciatic bone. Then, the inner tube-conductor is pressed on the fixing part of the wire and the fixing part of the wire is transferred to the designated position, due to which the fixing part is hooked to the inner surface of the acetabular pillar. Next, the main wire is pulled and the maximum approach of the pillar to the adjacent wall of the ring is achieved. Then, without loosening the pull on the wire, it is passed through the inner channel of the fixing screw. The latter is screwed into the drilled channel in the posterior pillar of the acetabulum. Then, without loosening the pull on the wire, the latter is passed through the internal channel of the screwdriver, and the screwdriver is inserted into the head of the fixing screw and, without loosening the pull on the wire, the screw is screwed until the screw head stops in the wall of the ring. After the reposition carried out in this way, the pillars and fragments are fixed to the ring with fixing screws passed through all free holes in the ring.
EFFECT: method provides the most accurate open reposition of pillars and fragments of the acetabular cavity from the acetabular cavity side, which meets the requirements of sparing reposition, allows the fixation of pillars and fragments of the acetabular cavity from the acetabular cavity side by fixing pillars and fragments from the acetabular cavity with the help of a repositioning-fixing support ring and guide tubes.
