FIELD: ophthalmology.
SUBSTANCE: transscleral coagulated laser action on the tumor is performed. Preliminarily, an endoscope is introduced through the incisions of the conjunctiva and the tenon membrane of the sclera, designed to search, observe, illuminate and influence the tumor with a helium-neon laser with λ = 0.63 μm, which is first carried out along the border of the tumor with a helium-neon working laser with λ = 1.06 microns with an irradiation power of 3 MW and a fiber spot diameter of 0.5-1 mm. Then, laser irradiation of the tumor is carried out over its entire area with a working laser with the same wavelength and diameter of the light spot with a radiation power of 5 MW, scanning the tumor within the border of the barrier effect on it until complete thermal destruction within 30-90 s until it turns pale, while observing the state of the tumor in the endoscope. At the same time, when irradiation is carried out in hard-to-reach places in the folds of the choroid of the ciliary body, a light-guide instrument is applied, which has a bend in the working part equal to the radius of the eyeball, and a bend of the distal end of the light guide with a radius of curvature of 7-10 mm, as well as a focusing microlens at the end of the light guide in the form of a sphere with a diameter of 1.5 mm. In a particular case, with a tumor of the ciliary body no more than 5 mm in height, the latter is affected transsclerally through its projection on the sclera with a light guide instrument having a bend in its distal part along the scleral radius.
EFFECT: method allows increasing efficiency of thermal destruction of the tumor, as well as to exclude the development of metastases.
