FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to traumatology and orthopedics, and can be used to treat deforming arthrosis of the first carpometacarpal joint of the hand. The first carpometacarpal joint of the hand (hereinafter referred to as the joint) is opened, the trapezium bone and chondromic bodies are removed from the joint cavity. Osteophytes of the base of 1 metacarpal bone and the distal pole of the scaphoid are resected. A graft is formed from the radial wrist flexor tendon. From the radial side, half of it is isolated along its entire length, cutting off from the muscle part and preserving the area of attachment to the base of the metacarpal bone. Through the formed canal, the graft is carried out into the joint cavity, sutured along the entire length, sutured, placed in the interval between the scaphoid bone and the base of 1 metacarpal bone, sutured to the tendon of the radial flexor of the wrist. Then 1 metacarpal bone is suspended to 2 metacarpal bone. At the same time, through the base 1 of the metacarpal bone at an angle of 40-50 degrees to its longitudinal axis, the first guide wire is passed with a loop at the end with the help of which the ends of the thread with a button in its middle are passed through the bone. The button is installed from the dorsal-radial surface of the base of 1 metacarpal bone. Through the middle third of the diaphysis of the 2nd metacarpal bone at an angle of 40-50 degrees to its longitudinal axis, a second guide wire is passed with a loop at the end with the help of which the ends of the thread taken from the 1st metacarpal bone are passed through the bone. Then the ends of the thread are passed through the second button which is located on the dorsal-ulnar side of the middle third of the diaphysis of the 2nd metacarpal bone. It’s necessary to tie the ends of the thread on the second button so that the base of the metacarpal bone is at the level of the base of the metacarpal bone, and 1 finger is in the position of maximum abduction.
EFFECT: method provides elimination of pain syndrome, preservation of hand grip strength, absence of cosmetic defect in the form of shortening of length of 1 finger, absence of adduction contracture of 1 finger, preservation of a good functional result due to tendon interposition, fixation of the base of 1 metacarpal bone to the middle third of the diaphysis of 2 metacarpal bone, and also maintaining the length of the first ray and complete removal of the trapezium bone.
