SUBSTANCE: invention relates to medicine, namely to traumatology and orthopedics, and is intended for surgical treatment of elbow joint instability after chronic dislocations and fracture-dislocations of forearm bones. After posterior access to the elbow joint, the ulnar nerve is mobilized and retracted on an elastic holder, an autograft is taken - the tendon of the semitendinosus muscle or the tendon of the long palmar muscle. Drilling of two bone tunnels is performed: the first tunnel is formed in the distal metaepiphysis of the humerus, the second tunnel is formed in the proximal metaepiphysis of the ulna - through an elevated tubercle on the medial side of the ulna with an exit distal to the supinator crest on the lateral side of the ulna. One end of the graft is tucked in and passed through the tunnel of the humerus, starting from the medial side. The other end of the graft is carried out in the ulnar tunnel from the lateral side, forming a loop. The autograft is stretched in the position of 90 degrees of flexion in the elbow joint and the neutral position of the forearm. The ends of the autograft are sutured together with interrupted overlapping sutures from the lateral side.

EFFECT: method provides reduction of invasiveness of surgical intervention as a result of using a single posterior approach, absence of additional fixing implants, risks of damage to the ulnar nerve due to stabilization of the ends of the loop from the lateral side.
